Pediatrician explains: the real reason your toddler wakes at 3am (and why it is not a phase)

"People keep telling me it's a phase. I'm not sure a phase goes on this long, and nobody can tell me when it ends."
I had just spent 20 minutes with her son, who was two and a half and had come in about a rash.Somewhere in the middle of it she told me she hated him more than she had ever hated anyone, and then put her hand over her mouth.
She had been woken 4 or 5 times a night for 15 months.
But here is the question I asked her that she could not answer.
"Do you know the one thing parents overlook all the time, but is one of the biggest reasons their toddlers act that way?"
She stayed silent for a minute.She did not know.In 15 months, nobody had asked her.
What 15 months of this does to a person
These are literally her words, and if any of it sounds familiar you should know it is not a character flaw.
"I stand in his doorway at 3am shaking.
It isn't the cold.
It's resentment and guilt and anxiety all at once and it frightens me.
I stand there thinking there is something deeply wrong with me as a mother."
"By 9am I'm fine.
Reasonable, warm, patient.
By 3am I'm someone I don't recognize."

She is not unusual.In the largest set of parent accounts I have seen on this, 2,112 parents, here is what they reported, in order:
- •822 described depression, anxiety, or being put on medication.
- •804 admitted resenting their own child.
- •318 described daily guilt and asking themselves whether they were a good enough parent.
- •168 described their marriage coming apart.
The research is blunt about it.
- •Parental stress runs 3 times higher in parents with disrupted sleep.
- •Workplace stress runs 90% higher in parents whose child has a sleep problem.
And the decline in marital satisfaction after a baby is one of the most consistently replicated findings in the field, with sleep loss identified as a direct contributor.
She had lived every line of that list.Here is the rest of it, in her words.
"I've stopped seeing friends because I can't hold a conversation.My husband has been in the spare room for 7 months, just for now, and we keep score of who got up and how many times.There's been nothing between us since the spring.It isn't that we don't love each other.We're just never awake at the same time."
And then the thing almost nobody says out loud.
"Some nights I resent him.And then I hate myself for it by morning."

Nothing was wrong with her.
When you are woken repeatedly, you do not simply lose hours.You lose the deep and REM stages that only arrive late in an uninterrupted cycle, and those stages are what regulate emotion.Take them away for long enough and the part of the brain that produces the fight response stops being properly supervised.
She was not a bad mother.Her emotional brakes had been removed, one night at a time, for 15 months.
3 weeks after that appointment she emailed me.I will tell you what she said at the end.
Why everything you have tried has not worked
Before she came in, she had done all of this, in this order.
● Earlier bedtime. ● Later bedtime. ● Capping the nap. ● Dropping the nap. ● A reward chart she stopped caring about in 9 days. ● A gate he learned to climb. ● Blackout blinds. ● White noise. ● Walking him back 40 times a night. ● Then 11 nights of sleep training.

I paused for a minute.
Then I said, "I have good news and bad news."
"I'll start with the bad news.Unfortunately, there is no magic cure that makes your child's sleep problems go away instantly, and I will never promise you that.
However, we can drastically reduce two things.
- How often he wakes at night.
- How long it takes him to get back to sleep.
Now, looking at every single item on your list, I see one thing in common.
Every one of them targets your child's behavior. By behavior I mean what he does at bedtime and in the night.Crying, calling out, climbing out, refusing to lie down, needing you in the room.
Most parents automatically assume that this is the problem, because behaviors are visible.
However, the trouble with behavior work is that it does not stop the thing that woke him up.It just teaches him to stop calling for you.
It is like taking painkillers for a toothache.The pain goes for a few hours.The tooth is still rotting.
That is why it holds for a couple of weeks, and then a cold comes, or a vacation, or a regression, and you are back at the start doing the whole thing again.
And every fix you were given takes 2 to 4 weeks to prove.So you spend 15 months cycling through them, and every failure quietly confirms the worst thought you have, which is that the problem is you.
But it is not your fault.
And here's the actual good news.Once you understand the actual reason it keeps happening, the fix gets so simple that you will see why every parent ignored it completely."
"What's the reason, doctor?"
"Let me ask you something first.Have you ever watched him sleep all the way through the night?"
When a child sleeps, you can see him moving.
But most of the night, there are way more movements that we don't know happening because it's just too small to be seen
Those movements are not random.
He is moving, but his eyes are closed, so he appears to be sleeping.And that is why this is always overlooked.
But the reality is, he is not actually getting the deep sleep that is vital for emotional and physical repair.
And there is something that keeps triggering micro arousals all night long.
"What is a micro arousal, doctor?"
"A micro arousal is when his brain is dragged up out of deep sleep and left on the surface.That drag out is the micro arousal.
Those micro arousals build over time into an accumulated trigger, and that is what leads your child to wake in the middle of the night and not fall back to sleep for minutes, or even hours.
For your child, that has been happening a few seconds at a time, again and again, every night.
Do you remember the last time you woke up in the middle of the night for no reason, feeling like you had been hit by a truck?Foggy, short with everyone, everything too loud.
Your child feels 7 times worse than that at 3am, because a growing child needs 7.2 times more recovery out of his sleep than you do.
But the thing is, in the beginning, nobody ever checks the one place it starts."
So no wonder you were lost.Without knowing any of this, all you could do was sit on his floor in the dark asking why he is like this.Then the same again the next night, and the next, until you stopped counting.
He is not doing it on purpose, and it is not a phase.
He is just never truly asleep, because something in his crib, something simpler than you would believe, keeps pulling him out of the sleep that counts.
So what is pulling him out?
The one thing nobody thought to check
There was one question I had not asked her yet.
"What pillows has your child laid on so far growing up?"
"In the beginning he never had a pillow. Then it was a stuffed animal for a while.Then I used to fold a blanket up and put it under his head. Now he uses those regular pillows that are flat."
"Hmm.."
"What is it?"
In 2021 an experiment was run across 1,072 families with children between 2 and 5, to find out the biggest single cause of a child not sleeping through the night. They changed one thing at a time and counted what triggered the most micro arousals.
Surprisingly, the child's airway came out to be the highest reason.
And what made the biggest impact on that was what the child's head was resting on.
But what’s worse is that 94% of what is given to a child to lay their head on in a normal household limits a child's airway 72 times more than it does an adult's.
Whether it’s no pillow at all, regular pillows, flat pillows, stuffed animals, folded blanket. And even the ones sold as toddler pillows..
Every single one of them was found to trigger 12.7 times the micro arousals a child that age should be having.
And the reason every one of them fails comes down to a single thing.
The neck.
Your toddler’s neck is like a bendy straw.Straight, the drink comes up fine.Kink it in either direction and almost nothing gets through.
Now picture breathing through a drinking straw, and then pinching it halfway shut.You would expect to lose half your air.You lose nearly all of it.
That is what even a small bend does to a toddler's airway.An adult's is closer to a garden hose, so the same bend barely registers.

This is what it feels like to be him with anything that hes laying his head on at night (or even nothing)
- You fall asleep with your chin pushed all the way down against your chest, and it stays there until morning.
- You sleep on your side with your head tipped down toward the mattress, neck bent the whole night.
- Something soft is covering your face.
You would not last an hour in any of them.
So he moves, and lands wrong, and moves again.All night, 4 feet from where you were lying awake listening for him.
That is what is pulling him out.
She asked me what she was supposed to do.

What I told her at the door
She asked if it was ever going to stop.
I told her what I tell every parent who has been at this longer than a year. Do not change the routine again tonight.You have changed the routine eleven times. Change what is under his head, change nothing else, and look at him in the morning.
You will know by breakfast whether that was it. And if the first morning looks much the same, give it a few more nights before you decide.
